Information regarding field conditions, game schedules, picture day times and more will be available online at lombardparks.com/youth-athletics. TEAM FORMATION GUIDELINES For information on how teams are formed and friendship requests, please visit lombardparks.com/youth-athletics. Please note: requests for teams, players or coaches are not guaranteed. PARENT MANUALS Programs including youth basketball, soccer and t-ball have parent manuals available online that answer many frequently asked questions. AT A GLANCE All participants must fill out Lombard Park District's Youth Sports Information Form available online at lombardparks. com/youth-athletics. Partcipants must also purchase a game jersey for $20 at Sunset Knoll Recreation Center. Please note: Pre-K league participants do NOT need to purchase a jersey. League shirts will be provided prior to the first game for the Pre-K league. Participants registering after the 07/11 deadline will be placed on a waitlist. Requests for teams, players or coaches will not be guaranteed. Teams will be formed using Lombard Park District's Team Formation Guidelines. Grades 5 and up will combine with Glen Ellyn Park District and play some games at Ackerman Park in Glen Ellyn.
