GENERAL INFORMATION
Participants registering after the deadline will be placed on a waitlist. Withdrawals after team formation are subject
to a $10 processing fee. All participants in youth soccer, basketball, softball, and t-ball programs must complete the
Sports Information Form available online, upon registration. Information regarding field conditions, game schedules,
picture day times, and more will be available online at lombardparks.com/youth-athletics.
TEAM FORMATION GUIDELINES
All participants must fill out Lombard Park District's Youth Sports Information Form which can be found online at
lombardparks.com/youth-athletics. Requests for teams, players or coaches are not guaranteed. Teams will be formed
using Lombard Park District's Team Formation Guidelines.
UNIFORMS & JERSEYS
Basketball | Soccer
Participants must purchase a game jersey for $22 at Sunset Knoll Recreation Center or Madison Meadow Athletic Center.
Flag Football | High School Basketball | Pre-K Soccer | Softball | T-Ball
Participants do NOT need to purchase a jersey. League shirts will be provided prior to the first game. The cost of the
uniform is included with your registration.

Programs including youth basketball, flag football, soccer, softball, and t-ball have parent manuals available online
that answer many frequently asked questions. Please visit lombardparks.com/youth-athletics to review these manuals.
