Finley Road Lombard, IL 60148 If the registration office is closed, place the completed registration form and payment in a sealed envelope and leave in the drop box outside the main entrance. FINANCIAL ASSISTANCE All residents should have the opportunity to participate in leisure programs regardless of their ability to pay. To that end, the Lombard Park District has initiated a financial assistance fund for people in need. Financial assistance applications are available at the recreation center. The Park District offers limited financial assistance in the form of reduced or waived fees. INSURANCE The Lombard Park District does not provide medical insurance coverage for individuals who participate in District programs or use District facilities. To do so would make programs and user fees cost prohibitive. REGISTRATION INFORMATION Complete the registration form and signature line. Unsigned forms or incorrect payments will delay the registration process. Please make note of registration deadlines for specific programs. Make checks payable to Lombard Park District, and include a phone number on the check. There is a $20 service fee for any returned checks. RESIDENTS & NON-RESIDENTS Residents within the boundaries of the Lombard Park District must show proof of residency to qualify for resident fees. A Lombard address does not always indicate resident status because not all Lombard addresses are within Park District boundaries. Golfers must show their driver's license or a utility bill to prove residency. WAITING LISTS Depending on the program or event, waiting lists are created based on capacity and/or deadlines. We will contact you if you have been placed on a waiting list or missed a registration deadline. REFUNDS AND CANCELLATIONS Programs may be canceled or altered due to low registration, changes in staff or facility availability. You are entitled to a full refund for a program that has been canceled by the Park District. Participants may request a full refund before a program begins. Restrictions apply to adult sports leagues, food and beverages, and tickets to entertainment and sporting events.
