Payment Options 2020 - 2021 School Year AM Only (7:00 - 8:30 am) PM Only (Until 6:00 pm) AM & PM 2-Day Weekly $18 / wk $30 / wk $48 / wk 3-Day Weekly $27 / wk $45 / wk $72 / wk 4-Day Weekly $36 / wk $60 / wk $96 / wk 5-Day Weekly $45 / wk $75 / wk $120 / wk 10-Day Punch Card* $150 (Can be used for AM or PM) PROGRAM TIMES AM CARE DROP-OFF: 7:00 - 8:30 am PM CARE PICK-UP: After school until 6:00 pm CLUB OPTIONS FULL TIME: Five days a week, with AM only, PM only or AM & PM options. PART TIME: Select two, three or four days each week to attend. Days must stay consistent. OCCASIONAL: Purchase a 10-day punch card for morning or afternoon care. 48 hours notice of attendance is required. HOW TO REGISTER All students in Hammerschmidt, Madison, Manor Hill, Park View and Pleasant Lane are eligible for enrollment. Registration is in person at Sunset Knoll Recreation Center. Online, phone and fax registration are not available at this time. A completed registration form and a non-refundable $25 deposit per child is required at the time of registration. A credit card for automatic billing must be presented at the time of registration. Registration for part-time enrollment must maintain consistent days. Changes to part-time enrollment must have approval and will be subject to a $5 change fee per child per change. Lombard Park District is excited to announce the adoption of ePACT Network for better safety! ePACT is a secure emergency network that will be used to collect medical and emergency contact information electronically. Not only will ePACT replace paper forms, but it will also ensure we have a way to communicate with you in the event of illness, injury or larger scale emergency. The parents/guardian listed as the main contact will receive an email, following registration, with directions and a link to complete all required forms through the ePACT Network. The ePACT information must be completed before your child's first day of Club Rec. Club Rec is Lombard Park District's before and after school program designed to provide a safe, recreational experience for your children. Children in grades K-5 will enjoy fun, active and supervised activities before and/or after school. This includes time for homework, physical activity, snack and arts and crafts. Club Rec is hosted at the following District 44 Schools: Hammerschmidt, Madison, Manor Hill, Park View and Pleasant Lane. CLUB REC FEES FOR 2020-2021 A $25 non-refundable deposit is due at the time of registration each year. Fees can either be paid in full, monthly or weekly. 9 Monthly payments will begin 08/10–05/10 (no December bill) 36 Weekly payments will begin 08/12–05/12 and are billed for the week ahead - no bills will be charged the week prior to Thanksgiving, Winter Break and Spring Break. 10% discount for each additional child 5-day weekly fee includes half days and early dismissal days. It does not include days when school is not in session (holidays, institute days, etc.) Club Rec participants receive special discount pricing for our School Day Off program and camps. WE'RE HIRING! Lombard Park District is hiring for our Club Rec Before and After School Program. We're looking for Site Supervisors, Assistant Supervisors, Counselors and Junior Counselors.
